Updates to City Weekly’s doctor discipline database are up for the month of October. ---
Two new doctors were added to the database: Gregory Hoffman M.D., whose license is on probation for two years, and Karl R. Nicholes M.D., who voluntarily surrendered his license for five years.
Also there is an update to Dr. Steven Pack, who, in October 2010, surrendered his license and will be unable to reapply for four years.
